COBYS Post-Permanency Services

COBYS Post-PermannencyPost-Permanency Services (sometimes referred to as post-adoption services) are offered to ensure that families are provided the support they need to encourage long term permanency for children. All adoptive and guardianship families are eligible for Post-Permanency Services, whether or not they have adopted a child from the child welfare system or through COBYS. These services are free of charge to Pennsylvania families and are subsidized through the Statewide Adoption and Permanency Network (SWAN). Families self-refer for these short-term services, which are used to support the child and family after the adoption or guardianship has been finalized. Services include an assessment, case management, respite, and support groups.

Through case management, COBYS will help the family to identify strengths and needs, establish a family support plan, coordinate services to meet family=s needs, and facilitate collaboration between various service programs. Through local support groups, families can build relationships, form a community of mentoring, receive education and have socialization and recreational activities for their children. Respite provides families with planned breaks so they can return to caring for their child, rejuvenated and revived. An initial assessment provides the opportunity for COBYS and the family to develop a personalized family support plan to meet their specific needs and strengths.
